LS-208 Notice of Final Payment or Suspension of Compensation Benefits
LS-208 form is completed by your employer if your employer is terminating or suspending compensation or medical benefits under the Longshore Act. Generally the LS-208 requires that your employer provide an explanation as to why benefits are being suspended or terminated. Sometimes the explanation is expected, for example in situations when you have entered into a final lump sum settlement of your medical or compensation benefits with your employer. In such situations your employer under the LHWCA will complete an LS-208 to simply notify the Department of Labor that you will not be receiving any further compensation or medical benefits.
However, in most situations the LS-208 is completed by your employer if they are suspending or contesting for some reason your entitlement to medical and/or compensation benefits under the LHWCA.
