LS‑203 Employee’s Claim for Compensation

The LS‑203 is the first form to be completed by an employee who has been injured under the LHWCA.  It is essentially a single piece of paper which constitutes a claim being filed by the employee for compensation and/or medical benefits under the LHWCA.  This is the form that you should complete if you have sustained an injury and you need to make a claim for compensation or medical benefits against your Longshore employer.
