LS‑201 Notice of Employee’s Injury or Death

Your employer may provide you with an LS‑201 after you have sustained an injury or accident under the Longshore Act.  Under the LHWCA the purpose of an LS‑201 is simply to put you, as the employee, on notice by your employer that you may have certain rights under the Longshore Act.  The LS‑201 is somewhat similar to the LS‑202 but the 201 is generally provided to you as the employee by your employer whereas the LS‑202 is provided by your employer to the United Stated Department of Labor.  If you receive an LS‑201 from your employer, it is best to immediately contact an attorney or discuss the issue with the Department of Labor.  In general you will want to file an LS‑203 as soon as your employer provides you with an LS‑201.
